Thinking clearly · Chapter 8

How did Apollo 13 get back to Earth?

After the service-module accident, the crew used the lunar module as a lifeboat, looped around the Moon and used its engine to help return home. The astronauts and Mission Control conserved resources and prepared the command module for re-entry.1

The mission changed

The landing was abandoned. NASA describes the crew sheltering in Aquarius after Odyssey lost power and oxygen. The lunar module's engine helped bring them back; the command module was needed for the return through Earth's atmosphere.1

Getting home meant working with the resources that remained, not continuing the original mission. Chapter 8 concerns the room that lets people name the problem as it now is.

Competence needs a room that hears it

NASA describes sustained work by controllers and crew to bring the spacecraft home. That is coordinated problem-solving, not proof that improvisation always wins.1

The chapter places Apollo 13 beside Columbia. The Columbia board called for stronger independent technical authority and a learning organization. I do not reduce either mission to optimism or courage: what a team does with unwelcome evidence matters.2

A check you can use

  1. Say what has changed and which goals must now be dropped.
  2. Inventory the resources and constraints you have.
  3. Make the next check and its owner explicit.

How did Apollo 13 survive?

The lunar module kept the crew alive while the return was organized. Conservation, ground support and preparation of the command module mattered. NASA gives the sequence rather than attributing survival to one inspired moment.1
