The premise, in one paragraph

Smart people with good information still make bad calls - at Deepwater Horizon, at Theranos, in the 2008 crash. What Are We Missing? asks why, and gives readers PRISM: five moves to run before any decision, so the missing question gets asked while it can still change the answer. It is written by a civil engineer who has spent twelve years inside major projects where these decisions are made.
